Cheat Menu:

Go to Options -> Accessibility, turn CRT Shader on/off several times.
A voice will say "Access granted" - now when you go back to Options, 
the Cheat Menu will unlock.

I unlocked this on Hard Story mode, but you can do it on Normal. In order 
for the continues to appear, you must be on Normal or higher AND have room
for the continues to be available or they will not appear and instead a 
skull powerup worth 10,000 points will instead.

-=SPARE DYSON route=-
* Level 1: Obvious behind a melee breakable wall that the game points out.
* Level 2: At the start of the level, shoot the Skynet logo on the scaffolding 
  to your left, before the first dog enemy.
* Level 3: Behind a wall with the Skynet logo on top of the 3rd flame-throwing 
  platform.
* Level 4: Before the 2nd boss you run through a terminator factory. On the 
  bottom floor, run past the ladder through the wall for this extra continue.
* Level 5: At the bottom of a long ladder to the left behind an out of place 
  looking breakable wall.
* Level 6: Punch the jukebox, Fonzie!
* Level 7: Behind the first obstacle right after switching to the T-800.
* Level 8: At the end of the level on the last floor, hit the coffee machine 
  to pop this out.
* SPARE DYSON
* Level 9: On the 3rd floor, in the first room to the right of the health 
  pickup, shoot this computer/server until it blows up revealing the continue.
* Level 10: Shoot the back panel of the helicopter with the Skynet logo on 
  it to reveal this extra continue.
* Level 11: Towards the end of the level when you're running along conveyor
  belts with steam pipes over them, there's a ladder going down between two
  conveyor belts where you'll find the continue.
* Level 12: After beating the HK tank, there's a crate as you're going down 
  the elevator the tank came up on. Shoot the crate to get the continue.
* The achievement will unlock during the ending cutscene.
